Lone Star Family Health Center (LSFHC) has received a Community Centered Health Home Action Planning Grant from the Episcopal Health Foundation (EHF) in the amount of $25,000 to pursue the implementation of a Community Centered Health Home (CCHH) model.
A CCHH is a healthcare organization that acknowledges that factors outside of medical services affect patient health outcomes and actively participates in improving them. The CCHH initiative was taken up as a funding priority by EHF in 2016.
“Better community health starts outside of our clinics,” says LSFHC Director of Development and Communications, Flora Davis, “By becoming a CCHH, we hope to identify specific health issues that our doctors see regularly, and then work with our community to help address these issues.”
LSFHC is already taking ‘out of the box’ approaches to health care through efforts like its Nature Explore® Classroom which encourages children to get outside to help combat common childhood health issues such as diabetes, ADD and ADHD and childhood obesity. The EHF grant will allow the non-profit health center to pursue additional innovative approaches to community health.
“We know that getting outside of our clinic walls is how we can best impact our community’s health,” says Davis, “We believe in one family, one community at LSFHC, and we will be furthering that belief through this process.”
The process that Davis refers to begins with a site visit with EHF and then research and outreach to determine what aspects of the community’s health LSFHC will help address. This action planning grant will begin a three year process for LSFHC. LSFHC will apply for funding through EHF totaling one hundred thousand dollars annually over three years to fully implement the CCHH model.
